Forsyth Registration
You are registering for the Forsyth camp on June 16-20 at Joint Venture Park
Forsyth County Parks & Recreation requires registration directly through their office. The three forms of registration (walk-in, fax and mail-in) are described on the Registration Information below.
Forsyth County Parks & Recreation Camp Registration Instructions
Payment accepted: Cash (walk-in only), Personal Check, Money Order or Credit/Debit Card (Master Card, Vias, AmEx, Discover).
Mail-in: Mail a registration form and payment to Forsyth County Parks & Recreation, P.O. Box 2417, Cumming, GA, 30028. No cash!
Walk-in: Visit the Forsyth County Parks & Recreation Dept. Monday - Friday, 8:30 am - 5 pm at these locations: Central Park Administrative Office, Sharon Springs Park Office, or Sawnee Visitors Center.
Fax: Fax in a registration form and use a credit card/debit card for payment. Fax is: 770-781-2221
Online: Visit www.forsythco.com. If you have not registered with Forsyth County Parks & Recreation before, you need to complete a registration form and fax, mail or drop off at one of the Forsyth offices.
Note: After completing and turning in the form below, please email camper's name and age to camp@tavanisoccer.com, in order for us send camp updates.
